Background: The aim of this study was to determine the effectiveness of acceptance and commitment therapy on depression, self-esteem and body image concerns, after the first delivery is women in Kashan city.

Methods: A quasi-experimental design with pre-test and the post test and control group with follow-up meeting is. For this purpose from among all women after the birth city of Kashan, using convenience sampling, 87 patients were screened. Of them, 30 were selected, and randomly placed in two groups: experimental and control. After a two-month follow-up session was conducted. At the pre-test and the post-test questionnaire to collect information from the Edinburgh Postnatal Depression Scale, Self Esteem Inventory and body image concern inventory was used. The treatment plan acceptance and commitment for 1 month 8 sessions of 90 minutes was performed and after the implementation of the post-test data using analysis of covariance with repeated measures and were analyzed.

Results: The results showed that between pre-test and post-test experimental group and control group and the results of follow-up session there is a significant difference.

Disscussion: According to the results obtained we can conclude Acceptance and commitment therapy to reduce depression and increase self-esteem and concerns about body image affects women after the first delivery.
