بررسی اپیدمیولوژی سوختگی در کودکان استان آذربایجان غربی

<p class="MsoNormal" dir="rtl" style="text-justify: kashida margin: 0cm 0cm 0pt text-align: justify text-kashida: 0%"><strong><span lang="FA" style="font-size: 9pt font-family: "B Traffic" mso-bidi-language: FA">مقدمه و هدف:</span></strong><span lang="FA" style="font-size: 9pt font-family: "B Traffic" mso-bidi-language: FA"> </span><span lang="FA" style="font-family: "B Traffic" mso-bidi-language: FA"><font size="2">شناسایی اپیدمیولوژی سوختگی و افراد در معرض خطر این ضایعه برای تعیین و تخصیص منابع و اتخاذ تدابیر پیشگیری و درمان ضروری است. در تحقیقی که تحت عنوان بررسی اپیدمیولوژی سوختگی در کودکان آذربایجان غربی صورت گرفته است، تلاش بر این بوده که ضمن تعیین علل و عوامل این مساله، راهکارهایی برای برطرف نمودن این عوامل و کاهش پی آمدهای ناشی از آن ارائه گردد.<!--stripped--><!--stripped--></font></span></p>
<p class="MsoNormal" dir="rtl" style="text-justify: kashida margin: 0cm 0cm 0pt text-align: justify text-kashida: 0%"><strong><span lang="FA" style="font-size: 9pt font-family: "B Traffic" mso-bidi-language: FA">روش کار:</span></strong><span lang="FA" style="font-size: 9pt font-family: "B Traffic" mso-bidi-language: FA"> </span><font size="2"><span lang="FA" style="font-family: "B Traffic" mso-bidi-language: FA">این پژوهش گذشته نگر در صدد بوده است تا کودکان زیر 15 سال را از نظر اپیدمیولوژی، مرگ و میر و عوامل ایجاد کننده مصدومیت های سوختگی در استان آذربایجان غربی در سال 1384 را با استفاده از پرونده های سابقه پزشکی در بخش سوختگی بیمارستان امام خمینی شهر ارومیه بررسی نماید. تعداد مصدومین 219 نفر بود. پس از گردآوری داده ها از نرم افزار آماری </span><span dir="ltr" style="mso-bidi-language: FA mso-bidi-font-family: 'B Traffic'">SPSS</span><span lang="FA" style="font-family: "B Traffic" mso-bidi-language: FA mso-ansi-language: FR"> استفاده گردید.<!--stripped--><!--stripped--></span></font></p>
<p class="MsoNormal" dir="rtl" style="text-justify: kashida margin: 0cm 0cm 0pt text-align: justify text-kashida: 0%"><strong><span lang="FA" style="font-size: 9pt font-family: "B Traffic" mso-bidi-language: FA mso-ansi-language: FR">نتایج:</span></strong><span lang="FA" style="font-size: 9pt font-family: "B Traffic" mso-bidi-language: FA mso-ansi-language: FR"><span style="mso-spacerun: yes"> </span></span><span lang="FA" style="font-family: "B Traffic" mso-bidi-language: FA mso-ansi-language: FR"><font size="2">5/33% از مصدوین دچار سوختگی در سال 1384 را کودکان زیر 15 سال تشکیل می دادند که گروه سنی زیر یک سال با 1/6% موارد بالاترین میزان سوختگی را تشکیل می دادند. سوختگی با آب جوش 3/63% کل موارد را شامل می شد. متوسط سنی مصدومین 15- 0 سال برابر 45/5 سال بود و نسبت کودکان پسر به دختر 73/0 به 1 بود. در 9/71% از موارد میزان سوختگی زیر 20% از کل سطح بدن و در 5/15% از موارد سوختگی بیش از 40% از کل سطح بدن را در بر گرفته بود.<!--stripped--><!--stripped--></font></span></p>
<p class="MsoNormal" dir="rtl" style="text-justify: kashida margin: 0cm 0cm 0pt text-align: justify text-kashida: 0%"><span lang="FA" style="font-family: "B Traffic" mso-bidi-language: FA mso-ansi-language: FR"><font size="2">مدت متوسط بستری در بیمارستان 74/7 روز بود که از 1 روز تا 60 روز متغیر بود. سوختگی با آب داغ عامل اصلی سوختگی (3/63%) بود که بدنبال آن سوختگی با شعله (8/29%) قرار داشت. متوسط سن کودکان دچار سوختگی با شعله 67/6 سال و متوسط کودکان دچار سوختگی با آب جوش 36/4 سال بود. مرگ و میر ناشی از آب جوش 50% و مرگ و میر ناشی از شعله 9/42% موارد را تشکیل می داد.<!--stripped--><!--stripped--></font></span></p>
<p class="MsoNormal" dir="rtl" style="text-justify: kashida margin: 0cm 0cm 0pt text-align: justify text-kashida: 0%"><strong><span lang="FA" style="font-size: 9pt font-family: "B Traffic" mso-bidi-language: FA mso-ansi-language: FR">نتیجه نهائی:</span></strong><span lang="FA" style="font-size: 9pt font-family: "B Traffic" mso-bidi-language: FA mso-ansi-language: FR"> </span><span lang="FA" style="font-family: "B Traffic" mso-bidi-language: FA mso-ansi-language: FR"><font size="2">در این پژوهش افراد مونث درمعرض بیشترین خطر سوختگی قرار داشتند و شدت سوختگی در کودکان بزرگسال بیشتر بود. یافته های این پژوهش مشخص کننده گروه های سنی در معرض خطر بیشتر و عوامل خطرزای موجب سوختگی در کودکان استان بود.سوختگی با آب جوش و شعله بیشترین عوامل ایجاد کننده سوختگی بودند. به نظر می رسد باید برنامه ای پیشگیرانه ، از جمله توجه به نکات ایمنی<span style="mso-spacerun: yes"> </span>در منازل و آموزش والدین کودکانی که در معرض خطر بیشتری قرار دارند تدوین گردد. مسئولین و سیاستگزاران نیز باید در انجام برنامه ریزی اساسی ضمن توجه به تدابیر پیشگیرانه و آموزش به والدین و اختصاص منابع انسانی و مالی، به نیازهای جسمی و روانی کودکان مصدوم توجه بیشتری داشته باشند.<!--stripped--><!--stripped--></font>
<p class="MsoNormal" style="margin: 0cm 0cm 0pt text-align: justify mso-layout-grid-align: none"><strong><span style="mso-bidi-language: FA">Introduction& Objective : </span></strong>Understanding the etiology of severe burns injuries and identifying high risk groups are<span dir="rtl" style="mso-bidi-language: FA"> </span>essential<span dir="rtl"> </span>for allotting resources for prevention and treatment. The objective of this study was to develop a<span dir="rtl" style="mso-bidi-language: FA"> </span>profile of severe childhood burns in Urmia city (the center of West Azerbaijan <st1:place w:st="on"><st1:placetype w:st="on">province</st1:placetype> of <st1:placename w:st="on">Iran</st1:placename></st1:place>). These<span dir="rtl" style="mso-bidi-language: FA"> </span>findings will hopefully<span dir="rtl"> </span>stimulate development of targeted and sustainable interventions for reducing burns<span dir="rtl" style="mso-bidi-language: FA"> </span>occurrence among identified pediatrics high-risk groups and help to guide health care efforts towards the<span dir="rtl" style="mso-bidi-language: FA"> </span>prevention of burn injuries.</p>
<p class="MsoNormal" style="margin: 0cm 0cm 0pt text-align: justify"><strong>Materials & Methods:</strong> This retrospective study which carried out (from 1st March 2005 to 1st March 2006) to find the<span dir="rtl" style="mso-bidi-language: FA"> </span>epidemiology, mortality and etiological factors of pediatrics burn cases in 0-15 year’s age group burn injuries<span dir="rtl" style="mso-bidi-language: FA"> </span>in province of West Azerbaijan in the North- West of Iran. Data from burn unit in Urmia was retrieved.<span dir="rtl" style="mso-bidi-language: FA"> </span>Demographic<span dir="rtl"> </span>characteristics, clinical features, and outcomes constituted our data file in Statistical Software,<span dir="rtl" style="mso-bidi-language: FA"> </span>SPSS.</p>
<p class="MsoNormal" style="margin: 0cm 0cm 0pt text-align: justify">Mean scare of enrich rest in employed women was 175.17<span lang="AR-SA" dir="rtl" style="font-size: 13pt">±</span> 24.47 and in housewives was 162.97 <span lang="AR-SA" dir="rtl" style="font-size: 13pt">±</span>26.81 (p<0.01). There was a significant difference between marital satisfaction , place of birth, housing status (p<0.05) and income (p<0.01), in employed women.</p>
<p class="MsoNormal" style="margin: 0cm 0cm 0pt text-align: justify mso-layout-grid-align: none">While this difference was significant between marital satisfaction , education level, education level and age difference (p<0.05) in housewives.</p>
<p class="MsoNormal" style="margin: 0cm 0cm 0pt text-align: justify"><strong>Results:</strong> Children who hospitalized with burns constituted 33.5% of all burn admissions. Infants (ages 0-1)<span dir="rtl" style="mso-bidi-language: FA"> </span>had<span dir="rtl"> </span>the highest prevalence (6.1 %). Scalds caused 63.3% of burns. The mean age of these children,<span dir="rtl" style="mso-bidi-language: FA"> </span>between 0 to 15 years, was 5.45, and male to female ratio 0.73 to 1. In 71.9 % of the patients burn extent<span dir="rtl" style="mso-bidi-language: FA"> </span>was less than 20% TBSA and 15.5 % suffered from >40% TBSA burns. The average length of stay in the<span dir="rtl" style="mso-bidi-language: FA"> </span>hospital was 7.74 days, varying from 1 to 60 days. Scald was the main cause of burn (63.3 %), followed by<span dir="rtl" style="mso-bidi-language: FA"> </span>flame (29.8%). Mean age of children with flame burns (6.67 years), was significantly higher (p < 0.001) than<span dir="rtl" style="mso-bidi-language: FA"> </span>those with scalds (4.36), or other causes. Gender had no influence on mortality.</p>
<p class="MsoNormal" style="margin: 0cm 0cm 0pt text-align: justify mso-layout-grid-align: none"><strong>Conclusion: </strong>Girls were found to be at greatest risk for a burn injury and older children were at higher risk for<span dir="rtl" style="mso-bidi-language: FA"> </span>severe burns. Prevention programs should target these high risk groups, with an emphasis on the unique<span dir="rtl" style="mso-bidi-language: FA"> </span>characteristics of each group.</p>
<p class="MsoNormal" style="margin: 0cm 0cm 0pt text-align: justify mso-layout-grid-align: none">Policy makers should reassess the benefits of a pediatric burn unit in Urmia. There is need for general<span dir="rtl" style="mso-bidi-language: FA"> </span>awareness through public education, which may lead to the prevention of significant number of such<span dir="rtl" style="mso-bidi-language: FA">
