Script' mso-bidi-language: FA"><span style="mso-spacerun: yes">  </span>ثبت یک جنبه حیاتی از کار پرستار و یکی از مهمترین وظایف اوست. ثبت پرستارباید کامل, دقیق ، به موقع و حقیقی باشد. از آنجا که هر گونه نقص و نارسائی در ثبت می تواند منجر به پیامدهای منفی وحتی عدم درمان گردد. ا ین مطالعه با هدف بررسی نحوه ثبت اطلاعات توسط کادر پرستاری در پرونده های بیماران بستری<span style="mso-spacerun: yes"> style="font-size: 11pt font-family: Nazanin mso-ascii-font-family: Tahoma mso-hansi-font-family: Tahoma mso-bidi-language: FA"> این مطالعه یک مطالعه توصیفی بود که روی150<span style="mso-spacerun: yes">  </span>پرونده بیماران بستری<span style="mso-spacerun: yes">  </span>در مراکز آموزشی درمانی علوی ، فاطمی وبوعلی <span style="mso-spacerun: yes"> </span>اردبیل<span style="mso-spacerun: yes">  </span>انجام شد. ابزار گرد آوری اطلاعات چک لیستی بود که با توجه به اهداف پژوهش طراحی شد. داده ها بعد از جمع آوری با استفاده از نرم افزار آماری </span><span dir="ltr" style="font-size: 11pt mso-bidi-language: FA">SPSS</span><span lang="FA" style="font-size: 11pt font-family: Nazanin mso-ascii-font-family: Tahoma mso-hansi-font-family: Tahoma mso-bidi-language: FA"> مورد تجزیه و تحلیل قرار گرفته درقالب<span style="mso-spacerun: yes">  </span>جداول آماری توصیفی ارائه شدند. <!--stripped--><!--stripped--></span></p> <p class="MsoNormal" dir="rtl" style="text-justify: kashida margin: Tahoma mso-bidi-language: FA">یافته ها:</span><span lang="FA" style="font-size: 11pt font-family: Nazanin mso-ascii-font-family: Tahoma mso-hansi-font-family: Tahoma mso-bidi-language: FA"> درپرونده های مورد مطالعه ، برگ نمودار علائم حیاتی 100%<span style="mso-spacerun: yes">  </span>موارد وجود نداشت. از 8 برگ که می بایست توسط پرستار تکمیل گردد، 7 برگ آن استاندارد بود و تنها در3% پرونده ها برگ جذب و دفع مایعات استاندارد نبود. متوسط ثبت پرستار در برگ پذیرش و خلاصه ترخیص2/26% ، در برگ مراقبتهای قبل از عمل به ترتیب برای پرستار بخش 47% و پرستار اتاق عمل 9% ، در برگ گزارش عمل جراحی70% ، در برگ مراقبت بعد از عمل به ترتیب عناصر اطلاعاتی <span style="mso-spacerun: yes"> </span>ورود به اتاق ریکاوری16% و عناصر اطلاعاتی خروج از اتاق ریکاوری</span><span lang="FA" style="font-size: 5pt font-family: Tahoma mso-bidi-language: FA"> براساس یافته های<span style="mso-spacerun: yes">  </span>حاضر حدود نیمی از داده ها توسط پرستاران ثبت شده بود . به نظر می رسد عدم آگاهی از <span style="mso-spacerun: yes"> </span>داده هاییکه باید توسط پرستار در پرونده ها ثبت گردد، بی توجهی نسبت به اهمیت ثبت، ضعف مدیریت<span style="mso-spacerun: yes">  </span>پرستاری ومدارک<span style="mso-spacerun: yes">  </span>پزشکی، وجود نقص<span style="mso-spacerun: yes">  </span>وابهام د رطراحی فرمها می تواند از عوامل تاثیر گذار در ایجاد این وضعیت<span style="mso-spacerun: yes"> Nurse's documentation must be complete, accurate and timely. Incomplete documentation can lead to negative outcomes in treatment and lack of treatment. <!--stripped--><!--stripped--></span></p> <p class="MsoNormal" style="text-justify: kashida margin: 0cm 0cm 0pt text-align: justify text-kashida: 0% mso-line-height-alt: 1.0pt"><span style="mso-bidi-language: FA">Goal of this study was assessment of documentation of information by nursing staff inpatient's files. <!--stripped--><!--stripped--></span></p> <p class="MsoNormal" style="text-justify: kashida margin: 0cm 0cm 0pt text-align: justify text-kashida: 0% mso-line-height-alt: 1.0pt"><strong><span style="mso-bidi-language: FA">Materials & Methods:</span></strong><span style="mso-bidi-language: FA"> This research was a descriptive study that was done on 150 inpatient's files in medical educational centers in <st1:place w:st="on"><st1:city w:st="on">Ardebil</st1:city></st1:place>. Seven forms from eight forms that must completed by nurse were standard. Average documentation nursing were in the admission and discharge summary sheet 25%, in the pre – operation care sheet for ward nurse<span dir="rtl"> </span>48% and for operation room nurse 9%. In the operation report sheet 69.3%. In the recovery room record for entrance items and Exit items 16.2% and 62% respectively. In the nurses note sheet rate of records was 96.3%, and in the vital signs control sheet 83.3%.<span style="mso-spacerun: yes">  </span>Total average documentation nursing was 51.2%.<!--stripped--><!--stripped--></span></p> <p class="MsoNormal" style="text-justify: kashida margin: 0cm 0cm 0pt text-align: justify text-kashida: 0% mso-line-height-alt: 1.0pt"><strong><span style="mso-bidi-language: FA">Conclusion:</span></strong><span style="mso-bidi-language: FA"> This study showed that half of data documented by nursing staff.
