fa مقایسه بکارگیری دو روش تمرینات تنفسی غیر فعال دق دستی و ارتعاش مکانیکی بر عملکرد قلبی و ریوی بیماران تحت تهویه مکانیکی در بخش مراقبت‌های ویژه Comparison of applying two passive respiratory exercises (manual percussion and mechanical vibration) on cardiopulmonary functions in mechanically ventilated patients in critical care units پرستاری Nursing پژوهشي اصیل Original Research مقدمه: تمرینات تنفسی غیر فعال به منظور جلوگیری از تجمع ترشحات، کلاپس ریه و بروز پنومونی مرتبط با ونتیلاتور در بخش‌ مراقبت‌ ویژه استفاده می شوند. مطالعه ای با هدف مقایسه اثر روش دق دستی و ارتعاش مکانیکی از تمرینات تنفسی غیر فعال بر عملکرد قلبی و ریوی بیماران تحت تهویه مکانیکی انجام گرفته است. روش کار: کارآزمایی بالینی تصادفی شده متقاطع با نمونه‌گیری در دسترس بر روی 21 بیمار تحت تهویه مکانیکی بستری در بخش‌ مراقبت‌‌ ویژه اجرا گردید. هر دو روش دق دستی و ارتعاش مکانیکی برای بیماران انجام و پارامترهای عملکرد قلبی و ریوی قبل و در چهار نوبت بعد از مداخله‌ ارزیابی گردید. داده‌ها با نرم افزار Stata 11 <span lang="FA" style="line-height: 105% font-family: 'B Traffic' font-size: 10pt mso-hansi-font-family: 'Times New Roman' mso-bidi-language: FA mso-ascii-font-family: 'Times New Roman' mso-ansi-font-weight: bold">و آمار توصیفی و آزمون t<span lang="FA" style="line-height: 105% font-family: 'B Traffic' font-size: 10pt mso-hansi-font-family: 'Times New Roman' mso-bidi-language: FA mso-ascii-font-family: 'Times New Roman' mso-ansi-font-weight: bold"> زوجی تجزیه و تحلیل گردید.  یافته ها: بین تاثیر دق دستی و ارتعاش مکانیکی بر روی تعداد ضربان قلب، فشار خون سیستولیک و دیاستولیک، متوسط فشار خون شریانی، مقاومت راه‌ هوایی و میزان اشباع اکسیژن خون شریانی پس از مداخله تفاوت معنی‌داری وجود نداشت. بین تاثیر دق دستی و ارتعاش مکانیکی تنها بر روی کمپلیانس ریوی در دقیقه 15 پس از مداخله تفاوت معنی‌دار مشاهده گردید (044/0p=)، بطوریکه دق دستی کمپلیانس ریوی را بیشتر افزایش می داد. نتیجه نهایی: روش <span lang="FA" style="line-height: 105% font-family: 'B Traffic' font-size: 10pt mso-hansi-font-family: 'Times New Roman' mso-bidi-language: FA mso-ascii-font-family: 'Times New Roman' mso-ansi-font-weight: bold">دق دستی و ارتعاش مکانیکی عملکرد قلبی و ریوی را بهبود و عوارضی برای بیمار ایجاد نمی‌کنند. از هر دو روش با توجه به شرایط بیمار و امکانات بخش می‌توان استفاده نمود. هر چند ارتعاش مکانیکی از نظر افزایش کمپلیانس <span lang="FA" style="line-height: 105% font-family: 'B Traffic' font-size: 10pt mso-hansi-font-family: 'Times New Roman' mso-bidi-language: FA mso-ascii-font-family: 'Times New Roman' mso-ansi-font-weight: bold">ریوی نمی‌تواند بطور کامل جایگزین دق دستی گردد. Introduction & Objective: passive respiratory exercises are used to prevent from accumulation of secretions, lung collapse and the incidence of ventilator-associated pneumonia in ICUs. This study is aimed to compare the effects of manual percussion and mechanical vibrations on cardiopulmonary functions of mechanically ventilated patients. Materials &Methods: This randomized crossover clinical study was conducted on 21 mechanically ventilated patients hospitalized in intensive care units who were selected based on convenience sampling. Both methods of manual percussion and mechanical vibrations were applied for the patients.Cardiopulmonary function parameters were assessed before and four times after the intervention. Data was analyzed by Stata software version 11, descriptive and analytical statistics and paired t-test. Results: There was no significant difference between the effects of manual percussion and mechanical vibration on heart rate, systolic and diastolic blood pressure, mean arterial pressure, airway resistance and arterial oxygen saturation. However, there was a significant difference between the effects of manual percussion and mechanical vibration on pulmonary compliance at minute 15 after intervention (P = 0.044) that is, manual percussion increased pulmonary compliance more than mechanical vibrations. Conclusions: Both manual percussion and mechanical vibration improve cardiopulmonary function and do not cause complications for the patients. Both methods can be used according to the patient's condition and possibilities of units.
