تاثیر مشاوره گروهی مبتنی بر مدل اعتقاد بهداشتی بر رفتار تغذیه ای زنان باردار دارای اضافه وزن: یک کارآزمایی کنترل شده تصادفی

<p style="font-style: normal; text-align: justify;"><span style="font-size:14px;"><span style="font-family:IRANsans;"><span style="color:#ffffff;"><strong><span style="background-color:#4e5f70;">مقدمه:</span></strong></span><span style="color:black;"> اضافهوزن و چاقی مادر باردار مشکلی مهم در مراقبت دوران بارداری محسوب میشود. این پژوهش بهمنظور بررسی تأثیر مشاورۀ گروهی مبتنیبر مدل اعتقاد بهداشتی بر رفتار تغذیهای زنان باردار دارای اضافهوزن مراجعهکننده به مراکز بهداشتی درمانی کرمانشاه در سال ۱۳۹۷ انجام شد.</span><br>
<span style="color:#ffffff;"><strong><span style="background-color:#4e5f70;">روش کار:</span></strong></span><span style="color:black;"> این کارآزمایی بالینی در سال ۱۳۹۷ روی نود نفر زن باردار شهر کرمانشاه انجام شد. از بین مراکز جامع سلامت بهصورت تصادفی چهار مرکز برای انجام مداخله و چهار مرکز برای گروه کنترل تعیین شد. سپس، چهار جلسه برنامۀ </span><span style="color:black;">مشاورۀ گروهی </span><span style="color:black;">تدارک دیده شده روی گروههای مداخله به مدت یک ماه اجرا شد. </span><span style="color:black;">محتوای مشاوره شامل مشاورۀ تغذیه و آشنایی با هرم غذایی و اصول تغذیه صحیح، افزایش حساسیت به چاقی و عوارض آن، درک شدت معضل چاق، درک موانع و منافع رعایت رژیم صحیح غذایی و افزایش خودکارآمدی و درنهایت رسیدن به رفتار تغذیهای سالم بود. </span><span style="color:black;">ابزار گردآوری دادهها پرسشنامۀ محقق ساخته مشتمل بر اطلاعات جمعیتشناختی و مامایی، سازههای مدل اعتقاد بهداشتی و آگاهی و پرسشنامۀ رفتار تغذیهای بود. همچنین، دادهها با استفاده از نرمافزار </span><span dir="LTR"><span style="color:black;">SPSS</span></span><span style="color:black;"> نسخه ۱۶ تجزیهوتحلیل و سطح معنیداری کمتر از ۰/۰۵ در نظر گرفته شد.</span><span style="color:black;"><span style="font-size:12.0pt;"></span></span><br>
<span style="color:#ffffff;"><strong><span style="background-color:#4e5f70;">یافتهها:</span></strong></span><span style="color:black;"> میانگین سنی شرکتکنندگان در گروه آزمون (۶/۱۹) ۲۴/۹۱ سال و در گروه کنترل (۵/۶۰) ۲۷/۵۳ سال بود. بعد از کنترل اثر نمرات پیشآزمون در سازههای آگاهی، حساسیت درکشده، شدت درکشده، منافع درکشده، موانع درکشده، خودکارآمدی و رفتار تغذیهای، نتایج نشان داد که بعد از انجام مداخله، میانگین این نمرات در گروه مداخله بهطورمعناداری بیشتر از گروه کنترل است (۰/۰۰۱</span><span style="color:black;">=</span><em><span dir="LTR"><span style="color:black;">P</span></span></em><span style="color:black;">).</span><br>
<span style="color:#ffffff;"><strong><span style="background-color:#4e5f70;">نتیجهگیری:</span></strong></span><span style="color:black;"> یافتههای این مطالعه اثربخشی مشاوره براساس مدل اعتقاد بهداشتی را در ارتقای رفتار تغذیهای زنان باردار با شاخص تودۀ بدنی بالا نشان داد. پژوهشگران این مطالعه مداخلات مشاورهای براساس این مدل را توصیه میکنند؛ بنابراین، اجرای مداخلۀ مشاورهای براساس سازههای مدل اعتقاد بهداشتی میتواند نقش مؤثری در اصلاح الگوهای رفتار تغذیهای زنان باردار ایفا کند.</span></span></span></p>
<strong><span dir="LTR"></span></strong>
<p style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:Times New Roman;"><span style="color:#ffffff;"><strong><span style="background-color:#4e5f70;">Introduction:</span></strong></span> Overweight and obesity are a significant challenge in pregnancy care. This study was conducted to investigate the effect of group counseling based on health belief model on nutritional behavior of overweight pregnant women referred to Kermanshah health centers.<span dir="RTL"></span><br>
<span style="color:#ffffff;"><strong><span style="background-color:#4e5f70;">Methods:</span></strong></span> This clinical trial was performed in 2018 on 90 pregnant women in Kermanshah. Among the comprehensive health centers, 4 centers were randomly selected for intervention and 4 centers for the control group. Then 4 sessions of group counseling program prepared for the intervention groups were performed for one month. The content of the counseling included nutrition counseling and familiarity with the food pyramid and principles of proper nutrition, increasing sensitivity to obesity and its complications, understanding the severity of the obesity problem, understanding the obstacles and benefits of following a proper diet and increasing self-efficacy and ultimately achieving healthy eating behavior. The data collection tool was a researcher-made questionnaire including demographic and midwifery information, health belief model constructs, knowledge and nutritional behavior questionnaire. Data were analyzed using SPSS 16. Significance level was considered less than 0.05.<br>
<span style="color:#ffffff;"><strong><span style="background-color:#4e5f70;">Findings:</span></strong></span> The mean age of participants in the intervention group was 24.91(6.19) and in the control group was 27.53(5.60). After controlling the effect of pre-test scores on the constructs of knowledge, perceived susceptibility, perceived severity, perceived benefits, perceived barriers, self-efficacy and nutritional behavior, the results showed a significant increase after the intervention, in the mean scores of the intervention group (<em>P</em>=0.001).<br>
<span style="color:#ffffff;"><strong><span style="background-color:#4e5f70;">Conclusion:</span></strong></span> The findings of this study showed the effectiveness of counseling based on the health belief model in promoting the nutritional behavior of pregnant women with high body mass index. Researchers in this study recommend counseling interventions based on this model. Therefore, the implementation of counseling intervention based on the structures of the health belief model can play an effective role in improving the nutritional behavior patterns of pregnant women.</span></span></p>
