Background and  Objective: In order to reduce the adverse consequences of low fertility rate, it is necessary to take interventions.A study was conducted with the aim of determining the effect of counseling on the knowledge and attitude of having children in single-child couples referring to comprehensive health centers in Hamadan.

Materials and methods: An interventional study with a pre-test and post-test design was carried out in two groups of cases and controls in 2022. The research population consisted of couples with one child who referred to comprehensive health centers in Hamadan city, who were included in the study by stratified random sampling method. The total sample was estimated to be 92 couples. The tool for collecting information was a researcher-made
questionnaire and it was compiled in three sections: demographic, awareness, and attitude.

Results: The independent t-test showed that the average score of women's fertility knowledge in the two test and control groups after the intervention and one month after the intervention had a statistically significant difference. There was no statistically significant difference in the average score of the childbearing attitude of women and men in the two test and control groups before the intervention, after the intervention and one month after the intervention (P>0.05).
 
Conclusion: In general, the results of this study showed that counseling is an effective tool in increasing knowledge in the field of childbearing and improving attitudes over time. Planning to increase the fertility rate should be done in terms of short-term, medium-term and long-term plans.
