Background and Objective: Heart failure is increasing with the aging population and is associated with high mortality. The Orem self-care model, with its emphasis on patient empowerment, can play an important role in improving clinical outcomes. Therefore, this study aimed to determine the effect of implementing Orem's self-care program on health outcomes among patients with heart failure.
Materials and Methods: This quasi-experimental study was conducted at Khatam al-Anbia Hospital in Salmas, Iran, in 2025. The sample comprised 60 heart failure patients selected using convenience sampling and assigned to two intervention and control groups. Data were collected through demographic questionnaires, the Orem model needs assessment form, and a health outcomes questionnaire at pretest and three months after the intervention. The intervention group received four training sessions based on Orem's educational support system, and the control group received routine nursing care. Data were analyzed using the SPSS software (version 19) with central tendency and dispersion descriptive indices, t-test, chi-square test and Fisher's exact test. The Shapiro–Wilk test was also used to assess the normality of the data.
Results: In the intervention group, significant decreases in blood pressure, low-density lipoprotein, high-density lipoprotein, triglyceride, and glucose levels were observed (P < 0.001). In the within-group comparison of the control group, the mean health outcome score decreased; however, the difference was not statistically significant (P = 0.085). In the intervention group, health outcomes increased by more than 44 units, a statistically significant change (P < 0.001). In the intergroup comparison, no significant difference was found between the mean score of the control group before training and that of the intervention group (P = 0.075); however, after training, the difference was significant (P < 0.001). The regression coefficient showed a significant, direct correlation between health outcomes before and after the intervention (r = 0.33, P = 0.011).
Conclusion: Education based on the Orem self-care model improved health outcomes in patients with heart failure. Therefore, structured educational programs based on needs, especially the Orem model, can be an effective strategy to improve the quality of patient care.
